Relevance:

As a result of many years of clinical research based on the teachings of Academician A. M. Ugolev on membrane digestion, A. I. Parfenov proposed a new nosological form – "Enteropathy with impaired membrane digestion" (ENMP) – a disease characterized by insufficient synthesis of enzymes in the small intestine mucosa (SOTK). According to the International Classification of Diseases of the 10th revision, ENMP corresponds to the code K90.4.

The clinical picture of ENMP is largely similar to irritable bowel syndrome (IBS) – a common disease, the etiology of which is unknown, the pathogenesis is due to violations of visceral sensitivity and barrier function of the intestine, and treatment only in 30% allows for temporary improvement and only in 10% - persistent remission. In many patients with IBS, the activity of membrane enzymes in the SOTC is reduced and they may not have a syndrome diagnosis, but ENMP. The use of a low-carbohydrate elimination diet, drugs that have a cytoprotective effect on SOTK, increases the activity of intestinal carbohydrates (lactase, glucoamylase, maltase, sucrose), improves food tolerance and recovery.

A new direction in enterology makes it possible to reduce the economic costs of Russian healthcare for the examination and treatment of patients with IBS and to increase the level of diagnosis of intestinal diseases. .

The goal of the program is to improve the knowledge of specialists on the diagnosis of ENMP, its differential diagnosis with IBS and other intestinal diseases, the use of elimination diets and cytoprotectors in complex therapy and the evaluation of the results of therapy.

As part of the course, we:

  1. Let's analyze the issues of epidemiology, pathogenesis and pathophysiology of ENMP.
  2. We will study the diagnosis, differential diagnosis, and clinical / laboratory scales for assessing the severity of ENMP.
  3. We will analyze the existing means for the treatment of patients with ENMP, aimed at increasing the activity of the enzymes of the SOTK, maintaining a diet adequate to the enzymatic capabilities of the SOTK and preventing relapses of the disease.
