Theory and practice: how doctors pass certification in the personnel center of Depzdrav

More and more doctors are testing their knowledge and skills in the Personnel Center of the metropolitan Department of Health. Compared to last year, the monthly increase in specialists who have passed a professional assessment amounted to 80 percent.
On average, more than 1.6 thousand medical workers undergo competency testing every month. Over the last three months of the trial — more than five thousand doctors.
"The personnel center started working in May last year. Its capabilities are systematically expanding, including in conducting evaluation procedures. They are attended by doctors and residents of the Moscow healthcare system, as well as those who would like to join our team from other regions. The demand from the professional community for the passage of evaluation procedures is constantly growing. This is facilitated by the fact that the Personnel Center has unique technical equipment. Its doors are open to all medical professionals all year round and seven days a week. During the first 12 months of the personnel center's operation (from May 2021 to May 2022), 26 thousand evaluation procedures were carried out there. Over the past three months, their number has reached more than 40 thousand," said Anastasia Rakova, Deputy Mayor of Moscow for Social Development."

Currently, five types of assessment are carried out here, allowing to check the level of professional knowledge and competencies of medical workers. All interested specialists can go through the evaluation procedure for the assignment of the status of "Moscow doctor" and certification for obtaining or upgrading the qualification category. Graduates of residency and doctors after retraining courses have the opportunity to obtain primary specialized accreditation at the center in order to start working in their specialty. In addition, the Personnel Center conducts an unscheduled certification of employees at the request of a medical organization, as well as an assessment of professional competencies for employment in any city hospital or polyclinic.
To prove their high level of professionalism, doctors and residents need to pass tests. Before that, medical professionals must fill out an electronic application form on the official website of the Personnel Center and attach scanned copies of all necessary documents to it.
The test for medical professionals consists of three parts: computer testing, demonstration of practical skills in the specialty on simulation equipment, solving situational problems based on real clinical cases.
First, doctors pass the theory in the computer room of the Personnel Center. They answer questions randomly selected by a computer program from a general list of tasks. Those who complete this stage successfully proceed to demonstrate practical skills. They change into medical uniforms and find themselves in offices equipped with simulation equipment. It can be either a mannequin or a simulator simulating a patient, or medical equipment used in diagnostic examinations or surgical interventions.
At this stage, each specialist must show that he can provide emergency medical care and perform cardiopulmonary resuscitation. The rest of the tasks depend on the specialty. For example, endocrinologists examine a patient who may have osteoporosis, and surgeons diagnose a closed wound and perform routine and emergency operations.
The quality of the practical tasks is monitored by experts sitting behind the mirrored wall of the office. They make their conclusion: whether the health worker will be admitted to the final stage. The third part of the exam is solving situational problems based on real clinical cases. Medical professionals need to make a diagnosis based on known data about the patient and prescribe treatment. They must also demonstrate the ability to act in non-standard situations and teamwork skills. Then the experts make a verdict — whether the specialist coped with the task or not. Those who fail the exam can retake it in a few months.
After successful completion of the tests, doctors receive documents depending on the type of assessment — a certificate of accreditation, confirmation or upgrade of the qualification category, primary specialized accreditation. Those receiving the status of "Moscow doctor" are issued a certificate and a special badge of distinction. For this title, doctors working in the Moscow city health care system receive an incentive payment of 15,000 rubles every month for five years.
Medical workers of 26 medical specialties can undergo the professional knowledge assessment procedure at the Personnel Center. For example, internists, pediatricians, anesthesiologists and resuscitators, urologists, surgeons, infectious disease specialists, cardiologists and other specialists. In the Personnel Center of the Department of Health of the City of Moscow, medical workers receive continuing professional education. It also hosts evaluation and accreditation events for specialists of the capital's healthcare system. The center has 31 simulation stations equipped with specialized equipment to demonstrate the practical skills of a doctor. More than 1.1 thousand simulators, simulators, medical equipment and other tools are used to assess competence and training. The center's capabilities allow for professional assessment in 89 medical specialties out of 97 possible. The center carries out the selection of employees in all specialties for city polyclinics and hospitals.
