Irritable bowel syndrome with diarrhea. MCSC patient history
NewsPatient M., 26 years old, applied to the MCSC named after A.S. Loginov with complaints about:
- frequent loose stools (up to 9 times a day);
- periodic abdominal pain, which decreased after defecation;
- bloating and rumbling;
- weight loss of 4 kg in the last 6 months.
The first symptoms of the girl appeared in 2019 on the background of stress – her beloved pet died. Then she turned to the doctor at the polyclinic, who prescribed an examination, but no abnormalities in the condition of her gastrointestinal tract were detected. The specialist recommended that she drink a course of enzymes. While taking the drugs, the patient had diarrhea, which occurred spontaneously.
Since January 21, 2023, the girl has been experiencing prolonged stress again, and her well–being has deteriorated greatly - the pain in the lower abdomen has increased, defecation has become more frequent. When the symptoms became unbearable, the patient called an ambulance and was hospitalized in an infectious hospital with suspected acute intestinal infection. During the examination, no data confirming infectious pathology was obtained.
After discharge from the hospital, there was no improvement in her condition, and the patient went for a consultation to the laboratory of functional bowel diseases of the MCSC named after A.S. Loginov. Here she was diagnosed, which included:
- anorectal manometry to assess the motility of the colon and rectum;
- consultations of a neurologist and a psychiatrist, taking into account complaints of stress.
The patient was diagnosed with irritable bowel syndrome with diarrhea. Somatized disorder.
The laboratory specialists selected a complex therapy that had a positive effect. With repeated admission after 2 months, the girl's general condition improved: pain decreased, loose stools did not bother, anxiety decreased.
Irritable Bowel Syndrome (IBS) – a common disease of the gastrointestinal tract. More often it develops in young people or adolescents. Predisposing factors include stress.
The disease is based on a violation of the regulation of the "brain-intestine" axis, which is characterized by the development of intestinal hypersensitivity to the usual volume of intestinal contents. This is manifested by pain, diarrhea, bloating and an insistent desire to visit the toilet. Therefore, IBS is considered as a biopsychosocial pathology requiring the participation of not only a gastroenterologist, but also a psychiatrist.
