Immunodeficiency as a cause of pneumonia. The story of our patient

A 57-year-old patient applied to the pulmonology department of MCSC.

  • For a long time she was bothered by a cough with viscous sputum, and she also complained of weakness and increased fatigue.

Earlier, in a conversation with a doctor at the clinic, she said that she often suffers from otitis, sinusitis and bronchitis.

Since 2023, she had exacerbations up to 5-6 times a year, she was prescribed antibacterial and mucolytic drugs, and for a while it helped.

  • And in 2024, during the examination, changes in her lungs were revealed. The doctor diagnosed pneumonia and prescribed antibacterial therapy, inhalations, expectorants.

There was an improvement, however, during the control examination of the chest organs, doctors noted different dynamics: on the one hand, those areas of the lungs where there had been changes before (infiltration) decreased, but on the other, new foci appeared in other parts of the lungs.

  • Experts recommended to observe.

3 months after the pneumonia, the patient again complained of cough with sputum, fever and general weakness.

  • The examination showed: pneumonia again. The treatment helped, but the cough did not go away completely. And all the same, physical activity was hard for the patient, she quickly got tired. 

After she was consulted by a phthisiatrician, who doubted that the inflammation was caused by a certain infection.

  • But taking into account the fact that pneumonia occurred constantly, new foci of inflammation were found during examinations, the doctor recommended contacting the pulmonology department of the MCSC for further examination and clarification of the diagnosis. 

Our doctors conducted a comprehensive examination. As a result, bronchitis, endobronchitis and bronchiolitis were confirmed, as well as bilateral pneumonia, but viral and fungal infections were not detected.

However, in blood tests, doctors drew attention to a pronounced deficiency in the level of all immunoglobulins.

  • As a result, according to the results of these studies, doctors determined that the patient had an immunodeficiency, which caused recurrent infections of the respiratory tract and ENT organs.

In the department, the patient underwent special antibacterial, bronchodilator and mucolytic therapy. As a result, the cough and shortness of breath completely disappeared.

Now the patient feels much better and leads an active life. In the near future, doctors recommended that she undergo a consultation with an immunologist to determine further surveillance tactics.
