When making an appointment for a consultation in Consultation and diagnostic department or to resolve the issue of hospitalization inMoscow Clinical Research Center named after A. S. Loginov you must provide the following documents:
- passport
- OMS policy
- SNILS
- extract from the outpatient card (form 027 / y)
- referral from the polyclinic at the place of residence (form 057/u-04), signed by the attending physician and stamped by the medical institution.
