Dear patients!

We continue to provide full medical care. Today, patients with oncological diseases can make an appointment for consultations with specialists in our Center without sending form No. 057/U-04 from the polyclinic at the place of residence.

For an initial consultation, you need to have:

• passport;
• SNILS;
• OMS policy;
• an extract from the outpatient card (form 027/y).
