Headaches and depression? History of the MCSC patient

Patient M. went to the center for personalized medicine of the MCSC with complaints of frequent, almost daily headaches (different in nature and localization), anxiety and sleep disorders. Episodes of headaches occurred since childhood, but were observed only with emotional, intellectual stress-they were stopped by the use of analgesics.

From the age of 25, the patient paid attention to the fact that for several months the mood can be steadily reduced with unwillingness to do anything, lack of strength, rapid fatigue, sleep disorders, decreased appetite – during such periods, productivity fell sharply, the patient could not work, nothing brought joyful emotions. At such moments, I turned to general practitioners several times. However, the prescribed nootropic and vascular therapy was without effect.

In July last year, the condition in terms of mood deteriorated again – the patient felt depressed, unwillingness to do anything, sleep and appetite were disturbed. They also began to worry about headaches on a daily basis-different in nature and localization, analgesics helped for a short time. The patient went to a neurologist, but the prescribed treatment did not help. The condition deteriorated further – headaches, fatigue, and fatigue became more frequent.

After another visit to the doctor without any effect, the patient turned to the center for personalized medicine. During the treatment, laboratory and instrumental examinations were carried out, as well as expert consultations. In the process, she was also consulted by a psychiatrist. We noticed episodes of consistently low mood in the past and the current state – depression, tearfulness, unreasonable anxiety, disturbed sleep, rapid fatigue, decreased libido, a pronounced decrease in performance.

The patient's condition was classified as recurrent depressive disorder, a current episode of moderate severity with somatic symptoms (a variant of depression when the episodes are repeated). Headaches were considered somatic symptoms in the structure of depression. Antidepressant therapy was prescribed with a gradual achievement of positive dynamics: mood improved, the usual level of activity returned, sleep, appetite, and libido normalized. But the main thing is that the headaches completely disappeared during the therapy, and the condition was restored to the usual, comfortable level.

In this case, it is noteworthy that the episodes of depression began in the patient at the age of 25, but proceeded without treatment and subsequently became more severe with the addition of somatic symptoms – headaches.

It is important to note such a seemingly simple point as constant monitoring of the dynamics of the condition-the selection of therapeutic (i.e. effective) dosage, assessment of the condition and tolerability of the drug, the combination of drug treatment with psychotherapy, if necessary-all this allows you to adjust the therapy in time, partially avoid undesirable effects of drugs and not miss significant changes in the condition.
