Positron emission tomography combined with X-ray computed tomography of the whole body with PET/CT with [18 F]-FDG
PET/CT is a high-tech alternative method of radiation diagnostics, both complementing traditional imaging methods (CT, MRI, ultrasound, etc.) and, in some cases, fully replacing them. The main radiopharmaceutical for PET/CT used in oncology is [18F]-fluorodeoxyglucose or [18F]-FDG – radiolabeled glucose analog. Accumulation of [18F]-FDG in PET/CT in organs and tissues is proportional to the degree of glucose metabolism, which is elevated in the cells of most types of malignant tumors. Thanks to this, PAT/CT with [18F]-FDG allows you to identify the tumor process, determine its prevalence, plan and evaluate the effectiveness of treatment.
General indications for PET/CT with [18 F]-FDG in oncology and hematology:
- Differential diagnosis of benign and malignant tumors.
- Search for a primary tumor focus in the presence of metastases from the NGO (CUP syndrome) and in patients with paraneoplastic syndrome.
- Staging of the established tumor process.
- Evaluation of the effectiveness of treatment of the established tumor process.
- Assessment of the nature of residual changes on the background of the treatment.
- Detection of recurrence of the tumor process, including with an increase in the level of tumor markers.
- Selection of the most informative site for biopsy in the tumor focus.
- Planning radiation therapy.
Absolute contraindications for PET/CT with [18 F]-FDG does not exist, there are relative contraindications:
- pregnancy (the decision to conduct a study is made by a medical council);
- acute pathological condition;
- severe somatic condition, including a condition that does not allow to be in a static supine position for 60 minutes;
- infectious or inflammatory process in the acute (active) phase;
- hyperglycemia (blood glucose levels above 11 mmol/l);
- conditions that do not allow the study to be performed without violating the methodology (neuropsychiatric disorders, including claustrophobia, severe pain syndrome, etc.).
Time limits for PET/CT with [18 F]-FDG after medical manipulations:
- 5-7 days after the biopsy;
- 12 days after completion of chemotherapy for hematological diseases;
- 14 days (ideally 21 days) after completion of chemotherapy for solid malignancies (with a shorter interval between injections - as close as possible to the next administration);
- 8 weeks (2 months) after the volumetric surgical intervention;
- 12 weeks (3 months) after completion of radiation therapy (radiation therapy of the brain is not a restriction to the study for all nosologies);
- targeted, hormonal and immunotherapy does not affect the timing.
PET / CT is performed with intravenous administration of an iodine-containing radiopaque preparation in the absence of contraindications. It is necessary to provide the results of serum creatinine analysis (no more than 1 month old).

What documents are needed for PET/CT with [18 F]-FDG?
On the day of the study you need to have with you:
- passport;
- the original of the referral (on paper with four seals) – when recording within the framework of the compulsory medical insurance;
- the original of the MHI policy;
- the result of a blood test for creatinine - the validity of the tests is 30 days;
- the protocol of the medical commission or the protocol of the oncological consultation (for diagnoses specified in Appendix 1 of the DM Order No. 1149 dated 11/24/2021 (as amended according to DM Order No. 109 dated 02/11/2022) on routing patients to PET/CT according to the MHI of Moscow) – when recording within the framework of the MHI;
- data from previous studies in other medical institutions (in the absence of data in the EMIAS system), including conclusions (CT, MRI, ultrasound, PET/CT, SPECT/CT, scintigraphy, etc.), including on electronic media (CD/DVD discs or USB drive with DICOM files);
- if necessary, take with you: diapers, urological / gynecological pads (immediately before the study you will need to change to new ones).
