Daily pH impedance monitoring
Daily pH impedance monitoring allows you to identify all reflux in the esophagus and the height of their spread.
Indications for reflux monitoring:Strictly on an empty stomach, the last meal was 6 hours ago.
- It is mandatory to have EGDS for a period of 3-4 months (to exclude contraindications to the study, which are determined by the doctor of the laboratory for the functional diagnosis of diseases of the esophagus and stomach).
- Performing manometry of the esophagus to clarify the location of the sensors and the choice of probe.
- It is advisable to cancel all acid-suppressing drugs for a reliable assessment of the quality of reflux. The exception is those who have already been diagnosed and need to evaluate the effectiveness of therapy.
- Provide the results of testing for HIV, hepatitis B and C, syphilis (examination time is not more than 6 months).
Contraindications to the study:Neoplasms of the pharynx, larynx, esophagus, stomach;
- Varicose veins of the esophagus
- Strictures, diverticula of the esophagus
- Esophagitis grade C or D (to conduct a study after epithelialization or a decrease in the severity of inflammation).
- Severe diseases of the cardiovascular system, etc.
Description of the study:Daily pH-impedance monitoring is carried out using a probe located inside the esophagus and the upper part of the stomach.
The probe is inserted through 1 nasal passage after anesthesia of the nasal mucosa with lidocaine. Then, with the help of sips of water, it is pushed into the esophagus to the stomach. If necessary, the location of the probe is monitored using survey fluoroscopy. After the probe is installed, the cutaneous electrode is attached to the skin of the patient's chest with patches. The Gastroscan IAM device is hung on the neck over the shoulder and the study begins. It is important during the study not to follow a restrictive diet, but to eat foods that provoke reflux. They are individual for each person. You need to keep a diary during the study and record the symptoms. It is also important to assess the relationship of the symptom with reflux. It happens that there are not many reflux in the esophagus, and the connection with them is high. This indicates hypersensitivity of the esophagus.
The patient remains in the department, under the supervision of medical staff. We recommend wearing comfortable clothes (T-shirt, sweatpants). Men who have excessive chest hair should shave off a small patch of skin on the upper half of the chest in front.
If the patient has an allergy to the patch, then it is necessary to take with you the patch to which there is no allergic reaction.
