What documents are needed?

For consultation in In the MCSC Endoscopic Center, diagnosis and choice of treatment tactics you need to provide:

1. Referral in the form 057/u-04 with the seals of the medical institution that issued the referral (carried out only by electronic referral, paper is not required).
2. Identity documents (passport, MHI policy, SNILS).
3. Blood tests (for a period of no more than 30 calendar days): HIV, Wasserman's reaction and hepatitis B and C.
