Phimosis

Phimosis — the inability to expose the glans penis due to the narrowness of the foreskin.

Causes of occurrence:

  • Trauma of the penis, as a result of which it is possible to form scar tissue, leading to a narrowing of the foreskin (the so-called scar phimosis);
  • Inflammation of the foreskin of the penis (balanoposthitis), which also leads to scarring and phimosis, often accompanies diabetes mellitus.
  • Genetic predisposition to the formation of phimosis as a result of the insufficiency of connective tissue in the body, in particular its elastic component.

Clinical symptoms:
The main symptom is the inability to completely expose the glans penis. 
With severe phimosis, when the foreskin is greatly narrowed, there are complaints of a violation of urination – the urine stream is thin, the patient has to push when urinating. 
In cases of joining the inflammatory process, the occurrence of balanoposthitis - inflammation of the head and foreskin of the penis, typical complaints are pain in the head of the penis and foreskin, purulent discharge from the opening of the foreskin, enlarged lymph nodes, fever. 
With paraphimosis, there is acute pain in the pinched head, the head increases in size, turns blue. This is an urgent situation that requires emergency assistance.
In adult patients, there are complaints of a decrease in potency, as a result of a psychological reaction to painful sensations during sexual intercourse.
To establish a diagnosis, a clinical examination by a urologist is sufficient. 

Treatment of the disease is surgical.
The essence of surgical treatment is reduced to the radial excision of the foreskin (circumcision, circumcision) and the stitching of the outer and inner leaves together. The operation can be performed under both local and general anesthesia. After the operation, an aseptic dressing is applied. There is no need for inpatient treatment, and the patient is discharged with the recommendations of the attending physician.
