Cancer of the appendix. MCSC patient history

In the MCSC named after A. S. Loginov, patient Sh. at the age of 43, he was diagnosed with mucinous adenocarcinoma of the appendix.

In June 2021, she was urgently operated on by surgeons of the city Clinical Hospital for acute appendicitis. According to the results of histological examination, mucinous cancer of the appendix was diagnosed. The patient was sent to the MCSC named after A. S. Loginov. 

Taking into account the nature of the tumor spread on PET-CT and the intraoperative picture, The Surgeons of the coloproctology department performed the removal of the right half of the large intestine, the large omentum and part of the peritoneum.

After the operative stage, extended histological and immunohistochemical studies were conducted, on the basis of which a decision will be made on long-term treatment tactics, namely, intra-abdominal hyperthermic chemotherapy.

Cancer of the appendix refers to rare forms of tumors of the gastrointestinal tract, which accounts for up to 0.5% of all forms of colon cancer.

According to the histological structure, tumors of the vermiform process are divided into benign and malignant:

  • mucinous cystoadenoma;
  • neuroedocrine cancer of the appendix;
  • adenocarcinoma of the appendix (mucinous cancer, cricoid cell carcinoma of the appendix).

The incidence of mucinous adenocarcinoma of the appendix is at least 50% of cases of all malignant tumors of the appendix.

Most often, cancer of the appendix is detected during Emergency Operations for acute appendicitis, due to the lack of specific diagnosis and asymptomatic course. When patients are admitted to emergency surgical hospitals, only the vermiform process is often removed.

The main method of treating tumors of the appendix is surgery in the volume of right-sided Hemicolectomy and resection of the right colon with the removal of regional lymph nodes.

When the tumor of the vermiform process spreads in the abdominal cavity, patients undergo cytoreductive operations. They are characterized by the removal of both the primary tumor and the secondary manifestations of the disease. 

After the surgical stage of treatment, patients are prescribed intraperitoneal hyperthermic chemotherapy (HIPEC), which significantly improves the prognosis.
