Melanoma of the skin with intestinal metastases. MCSC patient's history

A 40-year-old patient applied to MCSC. She was harassed for 2 months:

  • enlarged and painful lymph nodes in the right armpit, 
  • abdominal pain not related to eating,
  • loose stools up to 2 times a day with an admixture of mucus,
  • as well as general weakness and an increase in temperature d about 38C. 

During the examination, the doctor drew attention to a flat brown formation with a light center, irregular shape and with uneven contours of about 1.6 cm on the patient's abdomen.

  • The patient said that she had had this mole since birth, but during the year she noticed a change in its color.

The doctor suspected melanoma and ordered additional studies:

  • There are signs of severe iron deficiency anemia in blood tests;
  • stool analysis for hidden blood turned out to be positive
  • No pathology was found on the endoscopic examination of the stomach and intestines;
  • Ultrasound and MSCT revealed an ileal tumor of about 10 cm.To clarify the diagnosis, doctors also took a biopsy from the right axillary lymph node.

 

  • As a result of laboratory tests, metastasis of pigmented melanoma was detected in the patient.At the first stage of treatment, the patient underwent surgery: resection of the small intestine with a tumor, excision of the skin melanoma of the anterior abdominal wall and removal of the right axillary lymph node (lymphadenectomy).

 

  • After histological examination, doctors made the final diagnosis: epitheliodnokellular pigmented melanoma of the skin with metastases to the small intestine and lymph nodes. Melanoma of the skin has a high risk of progression.

Taking this into account, as well as a pronounced immune reaction, at the next stage the doctors prescribed immunotherapy to the patient. Now she feels well and continues treatment at our Center.

Metastasis of skin melanoma to the small intestine can be detected during life in 1-4% of patients

  • And distant melanoma metastases in the first 5 years after surgery are detected in 88% of patients. Of course, the late diagnosis of this disease worsens the prognosis.When changing the color and size of pigment spots on the skin, experts recommend immediately contacting an oncologist for early detection of melanoma.
