Risk of thrombosis in patients with inflammatory bowel disease

Patients with inflammatory bowel disease (IBD) have an increased risk of risk of thromboembolic complications (Teo). They aggravate the course of the disease itself and increase mortality from Crohn's disease and ulcerative colitis.

K risk factors the development of thrombosis includes:

  • exacerbation of ulcerative colitis and Crohn's disease;
  • hospitalization;
  • compliance with bed rest;
  • reception of systemic glucocorticosteroids;
  • use of intravenous catheters;
  • genetic predisposition to thrombosis;
  • smoking;
  • taking oral contraceptives.

An important role in thrombosis is the presence of genetic defects that may not appear clinically under normal circumstances, but lead to increased blood clotting in exacerbation of chronic inflammatory bowel disease.

Awareness of the risks of feasibility studies, as well as the ability of doctors to quickly diagnose and treat are vital for the patient. 

In the Department of IBD treatment, the clinical condition of 1238 patients treated in 2019 was evaluated. In 112 patients (9.0%) clinically significant feasibility studies were revealed:

  • venous thrombosis of the lower and upper extremities;
  • thromboembolism of the branches of the pulmonary artery. 

These patients underwent analysis of blood coagulation genes and assessment of homocysteine levels. Of the 112 patients, 45 (40.2%) revealed genetic mutations that cause a predisposition to the development of thrombophilia and impaired folic acid metabolism.

After receiving the results of the study, patients are referred to a geneticist and hematologist for appropriate therapy. It will help prevent the development of formidable thromboembolic complications.

Thus, testing for the presence of genetic mutations and assessing the risk of developing pathologies of the blood coagulation system makes it possible to competently prevent thromboembolic complications in children.:

  • outpatient patients with exacerbation of IBD;
  • patients undergoing inpatient treatment with high disease activity.
