In March 2020, the MCSC named after A.S. Loginov was contacted by patient A., 32 years old with a diagnosis of Crohn'S disease. Doctors diagnosed the disease 3 years ago, and prescribed biological therapy. However, she continued to complain of diarrhea, abdominal pain, recurrent anal fissures. Several times a year she was treated in hospitals in Moscow, but there was no improvement in her well-being.

In the department of treatment of inflammatory bowel diseases MCSC named after A.S. Loginov, the patient underwent a comprehensive examination. During colonoscopy, severe inflammation (ulcerative defects) was detected in the ileal and large intestine. This indicated the ineffectiveness of biological therapy with the prescribed drug. Also, experts have identified pronounced perianal lesions:

  • multiple anal fissures;
  • strictures of the anal canal.

To determine the tactics of treatment, the doctors of the department held a consultation together with coloproctology surgeons, where they decided on the need for surgical treatment - the formation of a loop ileostomy. This is an opening in the abdomen, to which a section of the small intestine is removed for the outflow of feces. 

The operation was successful, after which the patient began to take therapy with a new biological drug. During the control examination after 6 months, doctors noted an improvement in the condition of the small and large intestines. Thanks to the treatment and the right drug, the girl managed to achieve remission of the disease. Today she feels well and continues to be observed in our Center.


Crohn's disease is an immune–inflammatory disease that can occur in any part of the gastrointestinal tract. Today there is no method of treatment that would completely cure the patient. Drug therapy should be carried out without interruption and if the therapy has no effect, and the patient's condition gradually or sharply deteriorates, it is necessary to discuss the possibility of surgical treatment.

Perianal lesions in Crohn's disease are often the first manifestations of the disease. In 50% of patients with perianal CD lesions, the ineffectiveness of traditional conservative therapy is noted, therefore, the treatment of patients should be carried out with the mandatory inclusion in the team of not only IBD specialists, but also coloproctologists.
